You would like to change your name because you already have a joint surname (= married name) and would like to add your maiden name or the name you had before the marriage (= place in front of or append to it).
If the married name already consists of several names, this declaration is not possible.
If the maiden name or the name used at the time of marriage consists of several names, only one of these names can be added to the married name.
The declaration is binding. The order of the names cannot be changed later.
The same applies to names used in a registered civil partnership.
Prerequisites
The declaration can only be made personally by the spouse who wishes to add his/her name to the married name.
Procedure
You can make a name declaration at any German registry office. However, the declaration only becomes effective when it is received by the registry office that keeps your marriage register. If your marriage is not recorded in a German marriage register, your local registry office is responsible for the effective receipt of the declaration. Only then can a certificate of the name change and, if requested, a new marriage certificate be issued. With such a document you can apply for a new identity document.

- Valid identity card or passport of the person making the declaration
- current marriage certificate or certified printout from the marriage register in the original
(available from the registry office where the marriage took place).
Note
- If the marriage was entered into at the registry office where the declaration on the use of name is to be made, the above-mentioned documents are not required.
- If the marriage took place abroad, a birth certificate as well as a translation and legalization of the documents may be required.
- Further documents may be required.
