You have the surname of only one parent (not the married name of your parents) as your maiden name and wish to have the surname of the other parent or a double name consisting of the two surnames of your parents.

You would like to use only one or some of these names as your maiden name instead of your maiden name, which is made up of several names.
Prerequisites
The new determination of the birth name that you acquired as a minor can only be determined once.
The new designation requires the consent of the parent whose name is designated as the new birth name or whose name is placed in front of or added to the previous birth name, unless the parent is already deceased.
A double name consisting of the two surnames of your parents can be used with or without a hyphen. This may only result in a maximum two-part double name.
If your previous surname became the married name in your marriage, your name change will only extend to the married name if your spouse agrees with your declaration.
Procedure
You can make a name declaration at any German registry office.
If you need an interpreter, please bring one with you to the appointment.
However, the declaration only becomes effective when it is received by the registry office that keeps your birth register.
If your birth is not recorded in a German birth register, the responsibility for the effective receipt of your declaration lies with your local registry office.
Only then can a certificate of change of name and, if requested, a new birth certificate be issued. With such a document, you can apply for a new identity document

- Valid identity card or passport of the person making the declaration
- Current birth certificate or certified printout of the original birth register (available from the registry office in whose district you were born)

- If your birth was registered at the registry office where the declaration of name is to be submitted, some of the above documents are not required.
- Further documents may be required.
