If a person dies in a hospital, nursing home or prison, the institution's operator is obliged to report the death.
In all other cases, the following persons are required to report the death in the following order:
- Any person who lived in domestic community with the deceased person.
- The person in whose home the death occurred.
- Other persons who were present at the death or have knowledge of it from their own knowledge.
Note: In the case of deaths in public or private institutions, other persons who were present at the death or have knowledge of the death also have the right to report the death. They are also required to provide any information that the hospital cannot.
When the death has occurred, the death must be established by a medical examiner. This is called a post-mortem examination. The doctor issues a death certificate, which is required for reporting the death to the registry office. The death is recorded there.
Procedure
You must report the death in person or in writing. Hospitals, retirement and nursing homes and other institutions report the death in writing.
If you have commissioned a funeral home, they will report the death for you.
Note: A doctor must certify the death. If there are indications that the death was not natural, he or she will notify the public prosecutor's office. In this case, the investigating authority will report the death to the registry office.
The registry office enters the death in the death register and issues the death certificate.
Required documents
- medical death certificate sheets A and B (non-confidential part) and sheets 1 and 2. (confidential part) in envelope
- valid identity card/passport of the deceased person
- valid identity card/passport of the person reporting the death
- registration certificate of the deceased's last residence
- Birth certificate or certified printout from the register of births: In the case of deceased persons born abroad, an international birth certificate (without translation) or a national birth certificate (with translation in German) is required.
- Marriage or civil partnership certificate of the last marriage or civil partnership
- Proof of dissolution of marriage or civil partnership (divorce certificate or death certificate)
- If the marriage was contracted abroad, an international marriage certificate (without translation) or national marriage certificate (with translation in German) is required.
In the case of foreign-language documents and judgments, a translation prepared by a sworn translator must be enclosed.

The death must be reported no later than the third business day following the death (Saturday is not considered a business day).
